Failure Mode Effect Analysis

Failure mode effect analysis is nowadays the key approach in Marine offshore sector for the identification of possible risk and the development of possible mitigation and/or solutions. It is a very flexible tool applied to any activity on a voluntary basis and often required by legislation in force.

The primary objective of FMEA study is to provide a comprehensive, systematic and documented investigation, which establishes if the effects of single component failure and/or common mode failures in the system related equipment would lead to life-threatening situation for the personnel, to unacceptable damage to the vessel or the environment or to loss of production.


The creation of FMEA study or its verification is performed in two main stages: a theoretical phase and, consequently, a confirmation of the outputs of said analysis by means of onboard tests.
