Failure Mode Effect Analysis

Failure mode effect analysis is nowadays the key approach in Marine offshore sector for the identification of possible risk and the development of possible mitigation and/or solutions. It is a very flexible tool applied to any activity on a voluntary basis and often required by legislation in force.

The primary objective of FMEA study is to provide a comprehensive, systematic and documented investigation, which establishes if the effects of single component failure and/or common mode failures in the system related equipment would lead to life-threatening situation for the personnel, to unacceptable damage to the vessel or the environment or to loss of production.

Deliverables

The creation of FMEA study or its verification is performed in two main stages: a theoretical phase and, consequently, a confirmation of the outputs of said analysis by means of onboard tests.

The above activities will be carried out on the basis of the following flow:

  • kick-off meeting
  • reception of the drawings and technical document (in electronic format) needed for preliminary assessment
  • RINA to review the documentation submitted for Workshop discussion
  • workshop via Video Conference Tool or at customer office
  • RINA to review the documentation needed for proving trials
  • proving trials of the systems that are subject of the FMEA study
  • RINA to provide draft report of the study or its verification (Rev. 0 – Draft version)
  • closing remarks and implementation of comments (if any)
  • RINA to issue final FMEA report or its certification.
